What is Subsys?
Subsys is an opioid medication containing fentanyl. It is only prescribed to people who are opioid-tolerant and experiencing breakthrough pain even when taking other opioid drugs around-the-clock for cancer pain. Subsys comes as a liquid solution that is sprayed and absorbed under the tongue.
How do I take Subsys?
You will need to remain under your doctor’s care while take Subsys, since misuse of this drug can lead to addiction or a potentially fatal overdose. Follow your doctor’s instructions carefully. Depress the nozzle on the medication bottle to spray a dose of the Subsys drug into your mouth, and hold the liquid under your tongue for at least 30 seconds. If you are still experiencing pain 30 minutes after your first dose, you can use a second. Do not use Subsys more than four times a day.
Before taking Subsys, tell your doctor if:
- You are allergic to fentanyl or any other medications
- You are taking any other prescription or non-prescription medications or supplements
- You are unsure if you are opioid-tolerant
- You have a history or family history of substance abuse and addiction
- You have breathing problems or lung disease
- You have liver or kidney disease
- You have heart problems or low blood pressure
- You have any mouth sores or ulcers

What are the possible side effects of taking Subsys?
The most common side effects of Subsys are headaches, drowsiness, nausea, and constipation. Let your doctor know if any of these side effects are persistent or bothersome, or if you have any other side effects that worry you. Call your doctor right away in the event that you experience any serious side effects, including severe lightheadedness or drowsiness, unusual thoughts or behavior, difficulty breathing, or a sore throat. What if I forget to take a dose of Subsys?
The Subsys drug is taken on an as-needed basis to manage breakthrough pain, so missing a dose should not be an issue.
How do I store Subsys?
Store Subsys in its original packaging, in a dry space at room temperature. Even a single dose of Subsys can be fatal to a child, so keep your medication somewhere that children cannot access it. If you are no longer using Subsys, you will need to put it in the special disposal bags that came with your medication. If you have additional questions about how to safely dispose of leftover medication, talk to your pharmacist.
What happens if I overdose on Subsys?
Overdosing on Subsys can potentially be fatal and may cause extreme drowsiness, a weak pulse, and shallow breathing. If you or someone in your household has overdosed, call 911 immediately.
Warnings
Because the Subsys drug can cause dizziness and drowsiness, you should not drive until you know how the medication affects you. Do not drink alcohol while taking this medication; alcohol may cause dangerous and potentially fatal side effects. You should also avoid grapefruit products, as they may cause unwanted side effects. Keep in mind that Subsys can be addictive, and you should never use more than your doctor has prescribed.
