What is nifedipine?

Nifedipine is a dihydropyridine chemical, specifically a calcium channel blocker that is prescribed (either by itself or in conjunction with other medications) to treat severe chest pain (angina) and hypertension. Because nifedipine is a calcium channel blocker, it acts on the movement of calcium within the cells of blood vessels and heart, relaxing blood vessels and increasing the flow oxygen and blood to the heart. Topical nifedipine, when mixed with lidocaine, may also be prescribed as a cream or ointment for the treatment of chronic anal fissures or hemorrhoids.

How do I take nifedipine?

Both the capsule and the tablet forms of nifedipine are oral medications. Tablets are taken three or four times a day, depending on the prescription, while the extended-release capsules are taken once a day. The extended-release capsules should be administered on an empty stomach, two or three hours before or after a meal. Try to take nifedipine at the same time every day. If you were prescribed the topical nifedipine formula, gently apply the nifedipine cream or nifedipine ointment to the affected area according to the directives of the prescription.

Before taking nifedipine, tell your doctor if:

  • You are allergic to nifedipine or any of the ingredients in nifedipine medications
  • You are pregnant, planning a pregnancy, or are nursing a child
  • You are 65 or older
  • You are taking any medications (both prescription and non-prescription), vitamins, or supplements – especially St. John’s Wort
  • You have a narrowing or blockage of the gastrointestinal system, or have any condition that causes food to move slowly through the digestive system
  • You have COPD
  • You have congestive heart failure
  • You have had a myocardial infarction in the last fourteen days
  • You have, or have had, heart, kidney, or liver disease
  • You are taking anti-coagulants, Prandase or similar formulations of acarbose, antifungals, beta blockers, carbamazepine, Tagamet and similar medications, erythromycin, or digoxin
  • You are taking HIV protease inhibitors, diltiazem, metformin, quinidine, phenytoin, verapamil, rifampin, nefazodone, flecainide, or valproic acid

What are the possible side effects of taking nifedipine?

Some patients taking nifedipine have reported some side effects. Less severe nifedipine side effects include nausea, heartburn, and constipation; dizziness, headache, cough, cramping, rapid heartbeat, decrease in sexual capabilities, and flushing. However, should any of these conditions persist or worsen, contact your physician immediately.

Severe nifedipine side effects that require immediate medical attention include signs of an allergic reaction (hives, rash, difficulty swallowing, or swelling of the lips, mouth, tongue, throat, and lips), difficulty breathing, increased incidence of angina or similar chest pain, and signs of jaundice.

What if I forget to take a dose of nifedipine?

If you forget to take a scheduled dose of nifedipine, do so as soon as you remember and are able. However, if it is near to the time for the next one, skip the missed dose and resume the normal routine. Do not take multiple doses of this medication to make up for a missed one.

How do I store nifedipine?

Keep nifedipine tightly sealed in its original container, out of the reach of children and away from extremes of heat and humidity. If using one of the topical nifedipine medications, do not allow it to freeze. This includes both nifedipine ointment and nifedipine cream. Dispose of any unused or expired doses of this medication according to the recommendations of a pharmacist.

What happens if I overdose on nifedipine?

If an overdose of nifedipine is suspected, contact the Poison Control Center at 1-800-222-1222. If the individual has stopped breathing or lapsed into unconsciousness, contact Emergency Services at 911. Some of the symptoms of an overdose requiring immediate medical care include swelling of the extremities, nausea or vomiting, tachycardia, dizziness, and blurred vision.

Warnings

Do not drink grapefruit juice while taking nifedipine, and do not drink any for at least three days before starting the medication. Alcohol consumption can exacerbate the risks of serious side effects. This medication may cause a sudden drop in blood pressure (hypotension). Do not drive, operate heavy machinery, or use power tools until you know how nifedipine affects you.

Nifedipine is available under a number of brand names, as well as the generic. Brand names include Adalat, Adalat CR, Adalat CC, Procardia, Procardia XL, Nifedical CR, and Nifedical XL.
