What is fentanyl?

Fentanyl is an opioid analgesic drug that is used to treat severe or breakthrough pain. It is 100 times more powerful than morphine. It works similarly to other opioids by acting on the opioid receptors throughout the central nervous system to reduce sensitivity to pain. Fentanyl is more effective than morphine because it is more easily dissolved in fats or lipids. Because of this, Fentanyl is fast-acting and easily penetrates to the central nervous system. It is primarily prescribed as a transdermal patch for cancer patients who experience breakthrough pain. Breakthrough pain occurs when an individual suddenly experiences a burst of pain that shoots through the effects of other medications and then eventually subsides. Fentanyl is also used for anesthesia in operating rooms and intensive care units and for sedation during less serious procedures. Fentanyl patches may also be used to treat breakthrough pain resulting from other conditions besides cancer, such as broken bones, arthritis, and neuropathy.

How do I take fentanyl?

Buy fentanyl patches at your local pharmacy or drugstore with a prescription from your doctor. To use fentanyl patches, clean and dry the area of the skin where you will place the patch with water only. Do not use soap. Do not place the patch over areas of cut, broken, or irritated skin. Patches are usually placed on the abdomen. Do not cut or tear patches. Damaged patches may release excessive and dangerous amounts of medication into the body. Remove the patch from the pouch and remove the protective strip from the adhesive part of the patch. Do not touch the sticky part of the patch. Holding the non-sticky side of the patch in your palm, place the patch on the skin and press it down for 30 seconds. Wash your hands with soap and water before and after applying fentanyl patches. 

To remove the patch, peel it off and fold it in half so that the sticky sides are touching. Then flush the patch down the toilet. Patches disposed of in the trash could be found by children or pets and potentially cause harm. The patch is usually replaced every three days. Follow instructions given to you by your doctor. When changing your patch, place it on a different area of the skin each time.

Before taking fentanyl, tell your doctor if:

  • You have kidney or liver disease or problems going to the bathroom.
  • You have stomach or bowel problems such as IBS, constipation, Crohn’s disease, colitis, paralytic ileus or stomach pain.
  • You have a history of narcotic or alcohol abuse or dependence.
  • You are allergic to fentanyl or any of the other ingredients in fentanyl patches such as adhesives.
  • You have a mental disorder, depression or suicidal thoughts.
  • You have breathing problems such as asthma, COPD, or other respiratory conditions.
  • You have heart problems, low blood pressure, or an underactive thyroid.
  • You have an enlarged prostate or BPH.
  • You have stopped taking your regular pain medication.
  • You are taking prescription or nonprescription medications, herbal supplements or vitamins, especially St. John’s wort, antihistamines, cold or cough medicines, muscle relaxants, sedatives, and antidepressants.
  • You have ever had a serious head injury, brain tumor, stroke, or seizures.

Avoid alcoholic beverages while using fentanyl. Alcohol increases dangerous side effects. Do not drive a car or operate machinery. You may want to change your diet to avoid constipation while using this medication. Talk to your doctor about drinking grapefruit juice. It may affect the way this medication works.

What are the possible side effects of taking fentanyl?

Common side effects from taking fentanyl include diarrhea, nausea, dry mouth, extreme fatigue, confusion, constipation, weakness, and sweating. Less common side effects include anorexia, nervousness, hallucinations, depression, indigestion, retention of urine, anxiety, shortness of breath, and skin irritation or numbness from patches.

What if I forget to take a dose of fentanyl?

If you miss a dose of fentanyl or forget to change your patch, change it as soon as you remember. Do not use two patches unless your doctor has told you to do so.

How do I store fentanyl?

Store fentanyl in a cool, dry place inside of its protective pouch. Keep this medication in a place where no one will find it and use it, especially children or pets. Fentanyl could be fatal to them. Keep track of how many fentanyl patches you have at a time to be sure that no one has taken one. Dispose of patches by folding them in half and flushing them down the toilet. Patches in the garbage could be found and misused. Throw away packaging in the garbage. Wash your hands well after throwing patches away.

What happens if I overdose on fentanyl?

Signs of overdose on fentanyl include confusion, extreme sleepiness, difficulty breathing, loss of motor skills, pinpoint pupils, dizziness and coma. Overdose on fentanyl can be fatal. If you or someone you know has overdosed, contact emergency services immediately. If a child or pet has accidently used fentanyl, take them to the emergency room.

Warnings

Fentanyl is not safe for children under 18 years of age.  Do not drink alcohol, drive, nor operate machinery while using fentanyl. If you get a fever while using this medication, call your doctor. Fever may cause the body to absorb too much medication and lead to overdose.

Brand Names

Brand names for generic fentanyl transdermal patches include Duragesic. The generic fentanyl patch costs around half the price of Duragesic. Other names and forms of fentanyl include Actiq lozenges and Onsolis films that are placed inside the mouth.
