What is propylthiouracil?

Propylthiouracil is an oral medication that is used to manage hyperthyroidism due to an overactive thyroid gland. Propylthiouracil is most commonly prescribed to individuals with Graves' disease who did not tolerate methimazole, another type of thyroid medication, and are not able to receive surgery or radioactive iodine therapy. Propylthiouracil is also used to decrease symptoms of hyperthyroidism in preparation for surgical removal of the thyroid gland. 

How do I take propylthiouracil?

Propylthiouracil comes in the form of a tablet taken by mouth. It is usually taken three times a day, or every 8 hours.  Take propylthiouracil exactly as directed by your doctor.

Once your condition is controlled, your doctor may decrease your dose. Continue to take propylthiouracil even if you feel well. Do not stop taking propylthiouracil without talking to your doctor.

Before taking propylthiouracil, tell your doctor if:

  • You are allergic to propylthiouracil, any other medications, or any of the ingredients in propylthiouracil tablets
  • You take any other prescription or nonprescription medications, vitamins, nutritional supplements, and herbal products you are taking or plan to take. Specifically if you are taking any blood thinners such as warfarin, atenolol, labetalol, metoprolol, nadolol, propranolol, digoxin (Digitek, Lanoxin), or theophylline.
  • You have or have ever had leukopenia (decreased white blood cells), thrombocytopenia (decreased platelets), or aplastic anemia (condition in which the body does not make enough new blood cells), or other conditions that may affect red blood cells, white blood cells, or platelets
  • You have or had liver disease
  • You are pregnant, plan to become pregnant, or are breast-feeding
  • You are having surgery, including dental surgery

What are the possible side effects of taking propylthiouracil?

Side effects that require you to contact your doctor immediately include: sore throat, fever, chills, cough, or other signs of infection, headache, skin rash, hives, blisters, bumps or peeling, dark, rust-colored, brown or foamy urine swelling of the face, eyes, stomach, arms, hands, feet, ankles, or lower legs, chest pain, shortness of breath, wheezing, or coughing up of blood.

Less serious side effects that should still be mentioned to your doctor include: hair loss, difficulty tasting food, numbness, burning, or tingling of the hands or feet, joint or muscle pain, dizziness, or swelling of the neck.

What if I forget to take a dose of propylthiouracil?

If you miss a dose of propylthiouracil, take the missed dose as soon as you remember it. However, if it is almost time for the next dose, skip the missed dose and continue your regular dosing schedule. Do not take a double dose.

How do I store propylthiouracil?

Keep propylthiouracil in the container it came in, tightly closed, and out of reach of children. It should be stored at room temperature and away from excess heat and moisture. Discard any medication that is expired or no longer needed. 

What happens if I overdose on propylthiouracil?

In case of overdose, call your local poison control center at 1-800-222-1222. If the victim has collapsed or is not breathing, call local emergency services at 911.

Symptoms of a propylthiouracil overdose may include: nausea, vomiting, stomach pain, headache, fever, joint pain, itching, swelling of the hands, arms, feet, ankles, or lower legs

sore throat, fever, chills, unusual bruising or bleeding, blistering or peeling of the skin, numbness, burning or tingling of the hands or feet, yellowing of the skin or eyes, loss of appetite, pain in the upper right part of the stomach, excessive tiredness, or weakness. 

Warnings

Severe liver injury and acute liver failure have been reported in patients treated with propylthiouracil. Propylthiouracil should be reserved for patients who cannot tolerate methimazole or undergo surgery for the management of hyperthyroidism.
