What is doxycycline?

Doxycycline is a synthetic antibiotic derived from the broad-spectrum antibiotic, tetracycline. It is effective against a broad span of bacterial infections, including hemophilus influenza, streptococcus, pneumonia, chlamydia, gonorrhea, Lyme disease, acne, skin infections, genital and urinary infections, malaria, and acne. It works by interrupting the ability of the bacteria to produce its vital proteins to spread throughout the body. Doxycycline is only used for bacterial treatments and will not work for colds, flu or other viral infections. Doxycycline is anticipated to be an effective tool in the fight against bioterrorism because it is recommended by the Centers for Disease Control as a “drug of choice” for those with possible exposure to anthrax.

 

How do I take doxycycline?

Doxycycline is typically prescribed to adults, to be taken one or twice daily for between one and two weeks. On the first day of treatment, patients usually take 200mg (two doses of 100mg 12 hours apart) and then follow through the rest of their treatment with a single or divided daily dose of between 100 and 200mg. Doxycycline works best as an oral medication taken on an empty stomach at least one hour before or two hours after a meal. If taking doxycycline results in an upset stomach, it may be taken with food or milk but may not work as well.  You should not lie down for 10 minutes after taking your doxycycline dose, and you should wait at least two to three hours before or after taking any products containing aluminum, calcium, iron, magnesium, zinc, or bismuth subsalicylate. You should avoid antacids and dairy products, such as milk and yogurt, and also calcium-enriched juices, since these products will bind to prevent your body from fully absorbing the doxycycline.

If you have been prescribed doxycycline to prevent malaria, you should take your first dose one to two days before travel or as directed by your doctor. You should continue taking your doses while you remain within the malarious area and continue taking it for four additional weeks after you leave the area. Antibiotics, such as doxycycline, work best when the medication levels in the body are kept at a relative constant; therefore, you should space your doses as evenly as possible throughout your day. Do not stop taking this medication early. Doing so can allow bacteria to grow and may result in a return or worsening of the infection. Contact your doctor if your condition persists or worsens.

Before taking doxycycline, tell your doctor if:

  • You are allergic to doxycycline, minocycline, tetracycline, sulfites, or any other medications.
  • You are taking any prescription or nonprescription medications, particularly if you are taking antacids, anticoagulants, or vitamins. Doxycycline decreases the efficacy of oral contraceptives, and you should use another form of birth control while taking doxycycline.
  • You have or ever had diabetes, kidney disease, or liver disease.
  • You become pregnant, plan to become pregnant, or are breastfeeding. If you become pregnant while taking doxycycline, contact your health professional immediately. Doxycycline can have toxic effects on a fetus and should not be taken during pregnancy unless there is no other appropriate antibiotic.
  • You are having surgery, including dental surgery. You should also inform the doctor who will be performing the surgery that you are taking doxycycline.

What are the possible side effects of taking doxycycline?

Doxycycline is well-tolerated by the majority of patients who take it. However, common side effects include sore mouth, diarrhea, loose stools, nausea, abdominal pain, itching of the rectum or vagina, and vomiting. Doxycycline may also cause tooth discoloration in patients under 8 years old, and can result in higher instances of deep sunburn. Patients taking doxycycline for prevention of malaria have reported rashes in the sun-exposed parts of their bodies, but, of the patients who experienced this effect, the rash resolved upon discontinuation of doxycycline use.

Doxycycline crosses into breast milk but the effects of taking doxycycline on children under 8 years of age have not been documented on infants exposed through breast milk. While the consequent assumption is that the dose an infant would receive through breast milk is likely minimal, a potential risk theoretically exists.

The following are more serious side effects that should be immediately reported to your doctor and include severe headache, blurred vision, skin rash, hives, difficulty breathing or swallowing, redness of the skin (sunburn), yellowing of the skin or eyes, itching, dark-colored urine, light-colored bowel movements, loss of appetite, extreme tiredness or weakness, confusion, and decreased urination.

What if I forget to take a dose of doxycycline?

If you miss your dose of doxycycline, take it as soon as you can. If it is almost time for your next dose, simply take that dose and resume your prescribed dosing schedule. Do not double your dose or take extra doses.

How do I store doxycycline?

Doxycycline should be kept in the container in which it came. It should be kept in a tightly-closed container, out of the reach of children, at room temperature and away from excess heat or moisture. Any remaining medication after you stop treatment should be disposed of according to your doctor’s or pharmacist’s instructions.

What happens if I overdose on doxycycline?

If overdose is suspected, contact a poison control center or emergency room immediately. US residents can call their local poison control center at 1-800-222-1222, and Canadian residents can call a provincial poison control center. If the victim has collapsed or is not breathing, call local emergency services as 911.

Warnings

Doxycycline may cause excessive thinning of the blood and lead to exaggerated bleeding. It may also interfere with penicillins and should not be combined. Combining doxycycline and methoxyflurane may reduce kidney function. Doxycycline interacts with alcohol, and you should not consume alcoholic beverages while taking this drug.
